F-words in Childhood Disability’ Survey – International Service Providers

The purpose of this survey is to identify international service providers’ perspective on and experiences with the implementation of the ‘F-words’ (i.e., function, family, fitness, fun, friends, and future). This study is being conducted as part of an undergraduate thesis by Alice Soper under the supervision of Drs. Andrea Cross and Jan Willem Gorter.

There are 9 background information questions, such as your disciple and current residency. There are 14 questions rating the extent to which you agree with a statement and a question for you to write additional comments you may have. The survey is estimated to take 10 -15 minutes to complete.
